Job summary

AccentCare is seeking a Nurse Practitioner (Palliative Care) in Chicago for a PRN/part-time role. The NP will manage plans of care, perform assessments, and coordinate with physicians to optimize patient comfort and outcomes.

This on-site position covers the Downtown Chicago SNIF facility and requires NP licensure in the state of operation. Qualified candidates have NP certification, a valid DEA license, CPR, and willingness to travel to multiple care locations.

Qualifications

• Licensed to practice as a Nurse Practitioner in the state of operation.
• One year of NP experience preferred.
• Active CPR certification preferred.
• Must have a valid DEA license in the state of care.
• Must be a licensed driver able to travel to locations.

Responsibilities

• Develop and implement clinical plans of care for patients with chronic or terminal illness.
• Perform patient assessment, history and physical exams; advise on symptom management.
• Monitor progress and coordinate discharge planning with the team.
• Communicate with supervising and attending physicians regularly.
• Coordinate care with other staff and external agencies as needed.
• Prescribe medications as permitted by state boards.
• Support hospice-appropriate continuum of care and ongoing education.

What You Need To Know

Be the Best Palliative Care Nurse Practitioner You Can Be

If you meet these qualifications, we would love to meet you:

• Graduated from an approved school of professional nursing
• One year experience as a Nurse Practitioner is preferred
• Licensed to practice as a Nurse Practitioner in the state of agency operation
• Active DEA license in the state where care is being provided.
• Possess and maintain valid CPR certification while employed in a clinical role, preferred
• Must be a licensed driver who can travel to all business locations
• Meet the regulations and requirements of the state(s) in which program provides services

Responsibilities

As a Palliative Care Nurse Practitioner, you will:

• Develop and implement clinical plans of care for patients with chronic and terminal illnesses
• Perform patient assessment, history and physical; recommendations on aggressive symptom-management
• Monitor patient progress; update the Medical Director, Team Director and team members; consult regarding discharge plannning; and patient/family teaching
• Maintain communication with the supervisory physician (s) and attending physician to review patient records and care plans on a routine basis
• Initiate communication with attending physicians, other Palliative Care staff members and other agencies as needed to coordinate optimal care and use of resources for patient/family
• May provide support at time of death
• Prescribe medication as permitted by the state and/or the State Board of Nursing
• Participate in continum of care for patients that are hospice appropriate and ongoing education programs.
