Job summary

MVHC in Zanesville, OH is seeking a Nurse Practitioner to join the Transition of Care/Primary Care team. This 100% outpatient role offers collaboration with physicians, social workers, RNs, and LPNs in a supportive environment.

The NP will conduct post‑discharge visits, manage chronic diseases, perform medication reconciliations, and coordinate comprehensive care plans with emphasis on social determinants of health. This full‑time position includes benefits and no on‑call obligations.

Qualifications

• Master of Science in Nursing required.
• NP Certification required.
• CPR Certification required.
• One to two years of clinical experience preferred.
• Supervisory experience a plus.
• DOT Certification is preferred.

Responsibilities

• Conduct virtual Transition of Care visits within required post‑discharge timeframes.
• Perform comprehensive post‑hospital assessments and evaluate patients for ongoing medical needs.
• Complete medication reconciliation and address medication‑related concerns.
• Review hospital records, discharge summaries, laboratory results, and diagnostic testing.
• Develop and implement individualized care plans.
• Identify and address barriers to recovery, including social determinants of health.
• Provide chronic disease management and primary care services when appropriate.
• Order laboratory tests, diagnostic studies, medications, and referrals as clinically indicated.
• Full‑time with benefits
