Job summary

MVHC is seeking a Nurse Practitioner to join our Transition of Care/Primary Care team in Zanesville, OH. This full-time role focuses on outpatient care with no on-call requirements and shared responsibilities across medicine and care coordination.

You will conduct post-discharge transitions, perform assessments, and develop individualized care plans while collaborating with physicians, nurses, and social workers in a supportive, patient-centered environment. Epic EMR is used for documentation.

Qualifications

• Master of Science in Nursing required.
• NP certification required.
• 1-2 years clinical experience preferred.
• CPR certification required; supervisory experience a plus; DOT certification preferred.

Responsibilities

• Conduct virtual Transition of Care visits within post-discharge timeframes.
• Perform comprehensive post-hospital assessments and evaluate ongoing medical needs.
• Complete medication reconciliation and address medication-related concerns.
• Review hospital records, discharge summaries, labs, and tests.
• Develop and implement individualized care plans.
• Identify and address social determinants of health affecting recovery.
• Provide chronic disease management and primary care services when appropriate.
• Order labs, studies, medications, and referrals as needed.
• Full-time position with benefits, 40 hours per week.
