Job summary

Ennoble Care, in Baltimore County, MD, seeks an experienced Advanced Provider for full-time ambulatory home-based care, focusing on primary and palliative services for patients with chronic conditions.

You will work under physician supervision to deliver high-quality, patient-centered care and collaborate with our in-house clinical team to support diverse patient needs.

Qualifications

• Board Certified with valid NP or PA certification in Maryland.
• Strong clinical skills to assess, diagnose and treat.
• Positive demeanor and strong people skills.
• Ability to problem solve and operate independently.
• 1–2 years of primary care experience with geriatric patients.

Responsibilities

• Provide high quality primary and palliative care in and around Baltimore County.
• Perform physical examinations and update medical histories; develop care plans.
• Communicate with office staff across internal platforms.
• Develop impressions, diagnoses, and treatment plans.
• Order diagnostic testing as needed; prescribe medications and interventions.
• Consult with patients, families, and caregivers during visits.

Purpose:

We are looking for an experienced Advanced Provider in Baltimore County, MDregion on a full-time basis. You will apply medical techniques and principles under the supervision of a physician and you will care for a diverse set of patients with chronic diseases.

What You'll Do:

• Provide high quality care to primary and palliative patients in and around Baltimore County.
• Assess patient's health by performing physical examinations; obtaining, updating, and studying medical histories and creating the best plan for care.
• Effectively communicate with office staff and clinical operations across various internal platforms
• Provide high quality, patient centered care to a panel of patients
• Develop impressions, diagnosis, and treatment plans
• Foster trust and consult with patients, families, and caregivers during visits
• Order diagnostic testing as medically necessary
• Prescribe medications supplies and appropriate medically necessary interventions

What You'll Need:

• Board Certified, with a valid NP or PA certification in the state of Maryland
• Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose
• Positive demeanor and strong people skills
• Ability to confidently problem solve and operate independently
• 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)

Compensation:

• $120,000- $140,000+ (depending on experience and other incentives) (Full Time)
• $3000 Sign on Bonus
