This is a full-time role offering team-based care with the practice medical staff, care management, a psychiatric nurse practitioner and a licensed clinical social worker to deliver high-quality patient care.

Qualifications

• Active APRN/NP or PA license in Maine.
• Experience in primary care or hospital-employed outpatient setting preferred.
• Strong teamwork and communication skills.

Responsibilities

• Provide team-based care in collaboration with the practice medical staff, care management, a psychiatric nurse practitioner and a licensed clinical social worker.
• Provide daily patient care including new patient visits, transition of care, acute visits and more.
• Assist the triage team with clinical patient questions and concerns.
