Ideal candidates have PA/NP experience in primary care, a collaborative mindset, and a passion for compassionate patient service. New graduates with relevant experience will also be considered.

Responsibilities

• provide team-based care in collaboration with the practice medical staff, care management, a psychiatric nurse practitioner and a licensed clinical social worker.
• provide daily patient care including new patient appointments, transition of care, acute visits and more!
• assist the triage team with clinical patient questions and concerns.
• assist the medical staff with tasks related to patient care.
