Job summary

Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to provide high-quality primary care in senior living facilities. You will deliver direct patient care, focus on wellness, manage chronic conditions, and coordinate with other clinicians to ensure comprehensive resident care.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification
• Masters Degree in Nursing or Physician Assistant Studies
• Strong leadership, interpersonal, and problem-solving skills preferred
• Excellent written and oral communication skills
• Punctuality, organizational skills, and ability to function within a team

Responsibilities

• Perform comprehensive assessments, develop treatment plans, and provide ongoing care management for residents.
• Conduct regular rounds to assess residents’ conditions and adjust treatment plans.
• Collaborate with facility staff, family, and other healthcare providers for coordinated care.
• Educate residents, families, and staff on disease prevention and wellness.
• Maintain EMR documentation and ensure regulatory compliance.
• Participate in quality improvement initiatives to enhance care delivery.
