Job summary

Agape Care Group is seeking a Hospice and Palliative Care Nurse Practitioner in South Carolina to deliver compassionate, patient-centered care under physician supervision. The NP will diagnose, treat, and manage palliative care needs while supporting families through difficult health journeys.

Qualifications

• Graduate of an accredited masters, post masters, or doctoral nursing program with formal preparation as a Nurse Practitioner.
• Current unencumbered licensure as an APRN with designation as a Nurse Practitioner. Prescriptive authority with full DEA privileges.
• Must achieve and maintain national certification in the Nurse Practitioner specialty.
• Hospice/palliative care or chronic disease management experience preferred.

Responsibilities

• Provide diagnosis, treatment, and care for patients under physician supervision.
• Focus on delivering compassionate palliative care and supporting families throughout the care journey.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited masters, post masters, or doctoral nursing program with formal preparation as a Nurse Practitioner.
• Licensure: Current unencumbered licensure as an Advanced Practice Registered Nurse with designation as a Nurse Practitioner. Current prescriptive authority with full DEA privileges. Must achieve and maintain national certification in the Nurse Practitioner specialty
• Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice and/or Palliative background preferred.
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs.
