Job summary

Agape Care Group is seeking a Nurse Practitioner to join our hospice and palliative care team in South Carolina. You will provide patient diagnosis, treatment, and ongoing care under physician supervision, with a strong emphasis on compassionate palliative support.

You will work to ease patient and family burdens, deliver high-quality care, and collaborate with an engaged leadership team offering professional development and advancement opportunities.

Qualifications

  • Must have NP education and APRN licensure with NP designation.
  • Prescriptive authority with DEA privileges required.
  • National NP certification in specialty required.

Responsibilities

  • Provide diagnosis, treatment, and care under physician supervision.
  • Focus on hospice and palliative care.
  • Assist families with care plans and care journeys with empathy.

Qualifications
  • A heart to serve patients and families and a passion for providing the best possible care
  • Education: Graduate of an accredited masters, post masters, or doctoral nursing program with formal preparation as a Nurse Practitioner.
  • Licensure: Current unencumbered licensure as an Advanced Practice Registered Nurse with designation as a Nurse Practitioner. Current prescriptive authority with full DEA privileges. Must achieve and maintain national certification in the Nurse Practitioner specialty
  • Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice and/or Palliative background preferred.
  • Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs.
