Impact Health PC is seeking a compassionate Nurse Practitioner or Physician Assistant to join our clinical team serving patients in skilled nursing facilities in Lancaster, PA. You will deliver evidence-based, person-centered care with physician-level consult support and an autonomous yet collaborative practice.

The role offers a Monday–Friday schedule with no nights, weekends, or holidays, a robust benefits package, and ongoing clinical education. Licensure in good standing is required.

Qualifications

• Active NP or PA licensure in good standing.
• Minimum 2 years post-certification experience.
• Experience in SNFs, hospitalist, ED, or ICU preferred.
• Proficient with EHR systems and clinical technology.

Responsibilities

• Deliver person-centered care using evidence-based practices.
• Collaborate with interdisciplinary facility teams.
• Conduct assessments, develop treatment plans, and follow-up care.
• Educate patients, families, and staff; document within 24 hours.
