Job summary

Impact Health PC in West Hartford, CT is seeking a licensed NP or PA to deliver comprehensive medical and primary care to residents in skilled nursing facilities. You will assess acute and chronic conditions, develop treatment plans, and coordinate with physicians and care teams.

This role offers autonomy with strong clinical support, a Monday–Friday schedule, no nights or weekends, and earning potential up to $200k+ annually plus benefits including health, 401(k), PTO, and license/CE support.

Qualifications

• Active CT NP or PA licensure.
• Minimum 2 years post-certification clinical experience preferred.
• Experience in skilled nursing facilities, primary care, hospitalist medicine, emergency departments, or ICU preferred.
• Strong clinical assessment, communication, and organizational skills.

Responsibilities

• Provide comprehensive Medical/Primary Care to residents in skilled nursing facilities.
• Assess, diagnose, and manage acute and chronic medical conditions.
• Develop individualized evidence-based treatment plans and follow-up.
• Order and review medications, labs, imaging, and other tests.
• Collaborate with physicians, nursing staff, specialists, and care teams.
