Job summary

HarmonyCares is hiring Nurse Practitioners to deliver in-home primary care to high-acuity patients in multiple markets. This role emphasizes patient-centered care, not volume, with the support of an integrated, physician-driven care team. Travel to patient homes will be required in select markets.

Qualified NPs must be licensed in the state of practice, hold DEA eligibility, and have geriatric training or experience. A valid driver’s license and CPR/BLS certification are required.

• Licensed NP in the state where HarmonyCares Medical Group office located.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide primary care to high-acuity patients in home health and geriatrics.
• Practice medicine with focus on patient care, not volume.
• Take time to address patient needs and coordinate with the care team.
