Job summary

Enhabit Home Health & Hospice is seeking a Nurse Practitioner to provide compassionate hospice care and coordinate with an interdisciplinary team in a home-based setting. You will assess patients, manage pain and symptoms, and participate in care planning to ensure high-quality, patient-centered outcomes.

The role requires NP licensure in the practicing state, familiarity with Medicare conditions of participation, and solid technology skills for mobile devices and documentation.

Qualifications

• Must be a graduate of an approved school and licensed as a nurse practitioner (NP) in the state where they practice.
• Must have demonstrated knowledge of regulatory requirements, including Medicare conditions of participation.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, essential

• Must be a graduate of an approved school, and licensed as a nurse practitioner (NP) in the state in which they currently practice.

Education And Experience, Preferred

• At least five years nursing experience with a minimum of one year in hospice preferred.
• At least three years of clinical registered NP experience preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification
