Job summary

Enhabit Home Health & Hospice seeks a licensed Nurse Practitioner to provide medical care to hospice patients in a 50-55 mile radius from the Broken Arrow, OK office. Responsibilities include eligibility assessment and pain/symptom management as part of the interdisciplinary team.

Candidates should have an NP license in the state, plus knowledge of Medicare conditions of participation, and strong tech skills; hospice experience is a plus.

Qualifications

• Must have CPR certification.
• Must have knowledge of regulatory requirements, including Medicare conditions of participation.
• Must have basic technology skills including use of a mobile device.
• Five years nursing experience with at least one year in hospice preferred.
• Three years of clinical NP experience preferred.

Responsibilities

• Provide medical care to hospice patients.
• Determine hospice eligibility and pain and symptom management.
• Participate in the interdisciplinary team caring for hospice patients.

Responsibilities

Provide medical care to hospice patients. Determine hospice eligibility and pain and symptom management. Participate in the care of the hospice patient as a member of the interdisciplinary team

Qualifications

Education and experience, essential

• Must be a graduate of an approved school, and licensed as a nurse practitioner (NP) in the state in which they currently practice.
• Must have demonstrated knowledge of regulatory requirements, including Medicare conditions of participation.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education And Experience, Preferred

• At least five years nursing experience with a minimum of one year in hospice preferred.
• At least three years of clinical registered NP experience preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification
