Duties & Responsibilities

• Provide comprehensive, equitable, and effective medical care to a panel of Medicare patients.
• Conduct regular patient visits (average 13-16 per day), including routine, diagnostic, and new patient assessments.
• Develop and implement individualized care plans in collaboration with a multidisciplinary team.
• Utilize on-site laboratory services and access to specialist consultations through RubiconMD.
• Document patient encounters using EMR systems (Canopy and Greenway).
• Participate in preventive medicine initiatives and patient wellness activities.
• Ensure accurate documentation, including HCC, ICD-10 coding, and Health Risk Assessments where applicable.
• Engage in ongoing communication with patients, families, and care team members to ensure optimal outcomes.
• Support community engagement through patient activities such as exercise classes, wellness events, and educational programs.

Qualifications & Requirements

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of a Nurse Practitioner Certificate Program.
• National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
• Active, non-probationary Nurse Practitioner (NP) license in the state of Arizona.
• Active DEA license required.
• Bilingual proficiency in Spanish is preferred.
• Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments is a plus.
