This full-time opportunity offers a rewarding environment with a strong emphasis on preventive medicine, chronic disease management, and comprehensive patient support.

The Nurse Practitioner (NP) will work collaboratively within a care team, including scribes, medical assistants, and social workers, to ensure equitable access to healthcare and improved patient experiences. You will have access to modern facilities, on-site laboratories, and advanced electronic medical records systems to streamline your workflow and enhance patient care quality. As a Nurse Practitioner (NP), you will manage a panel of Medicare patients, averaging 13-16 visits per day, with a focus on comprehensive assessments every three months or more frequently for complex cases. You will conduct regular and diagnostic visits, collaborating closely with other healthcare professionals to coordinate care. The clinic is equipped with labs, phlebotomy services, and access to medical specialists via RubiconMD, ensuring patients receive timely and specialized care when needed. Patient transportation is provided within a five-mile radius, and the clinic hosts community activities to support holistic health and wellness.

Our organization is committed to supporting Nurse Practitioners through a competitive compensation package, extensive benefits, and a clear path for career progression. You will benefit from a physician partnership track, continuing education support, and a generous paid time off policy. Health, vision, dental, and life insurance, as well as 401(k) investment options with immediate vesting, are provided to support your financial security and well-being. Tuition reimbursement and medical malpractice insurance are also included as part of our comprehensive benefits plan.

The ideal candidate for the Nurse Practitioner (NP) role will hold an advanced nursing degree and national certification in a relevant specialty. Bilingual abilities, particularly Spanish, are highly valued to serve our diverse patient community. Experience with HCC documentation, ICD-10 coding, and health risk assessments is preferred but not required. This position is well-suited for individuals who are passionate about preventive care, value-based medicine, and making a meaningful difference in patients' lives.
