PHP Nurse Practitioners have a passion for delivering quality care to seniors and other residents in various healthcare settings, including skilled nursing facilities, assisted living, independent living communities, and via telehealth.

We are currently recruiting Nurse Practitioners for the Charleston Healthcare Center in Charleston, WV. Our Nurse Practitioners work Monday - Friday, 8am-5pm, with a salaried position that may require more than 40 hours some weeks. No call, weekends, or holiday coverage required. Must be willing to work 5 days a week.

Job Summary:

The PHP NP provides high-quality primary care in a person-centered model, diagnosing, treating, and managing acute and chronic illnesses, and collaborating with the care team and families to develop care plans. This approach aims to reduce hospital admissions and ER visits, emphasizing customer satisfaction.

Responsibilities include:

• Assess, diagnose, and manage conditions.
• Provide health education and develop treatment plans.
• Order and interpret lab and diagnostic tests.
• Document accurately in EMR.
• Collaborate with healthcare teams and support strategic initiatives.
• Ensure regulatory compliance and participate in family conferences.
• Coordinate care with other providers, including telemedicine.
• Complete required visits and documentation.
• Be present in assigned facilities during work hours to ensure adequate coverage.

Qualifications & Experience:

Required:

• Master of Science in Nursing, DNP, or NP Certification.
• National certification (AANP or ANCC).
• Active, unrestricted state license and DEA license.
• US work authorization.
• 2+ years clinical experience in geriatrics, adult, or family practice.
• Flexible schedule and commitment to senior and low-income populations.

Preferred:

• Geriatrics training or experience.
• Experience with ICD-10, HCC coding, and value-based care models.
• Experience in outpatient primary care and collaborative healthcare settings.
