Join to apply for the Nurse Practitioner (NP), Adult and/or Gerontology role at Personalized Health Partners.

Now offering a $20,000 Sign-On Bonus to successful hires (paid half at 90 days and half at 1 year).

Position Overview: PHP Nurse Practitioners are dedicated to delivering quality care to seniors and other residents in various healthcare settings, including skilled nursing facilities, assisted living, and telehealth. This full-time role is based at Lincoln Healthcare Center in Hamlin, WV, and Wayne Healthcare Center in Wayne, WV, with a schedule of first shift, Monday to Friday, no on-call, weekends, or holiday coverage.

Responsibilities include:

  1. Assess, diagnose, and manage chronic and acute conditions.
  2. Provide health education and develop treatment plans.
  3. Order and interpret diagnostic tests, adjust treatments accordingly.
  4. Maintain accurate documentation in EMR software.
  5. Collaborate with healthcare teams and support strategic initiatives.
  6. Ensure excellent patient experience and adhere to ethical and legal standards.
  7. Coordinate care with other providers, including telemedicine services.
  8. Participate in family conferences and provide advanced care planning.
  9. Ensure face-to-face coverage during work hours at assigned facilities.

Qualifications & Experience:

Required:

  • MSN, DNP, or NP certification graduate.
  • Active certification (AANP or ANCC).
  • Unrestricted state license and DEA license.
  • US work authorization.
  • At least 2 years of clinical experience in geriatric, adult, or family practice.
  • Flexibility in work environment and schedule.
  • Passion for healthcare for seniors and low-income populations.

Preferred:

  • Geriatrics training or experience.
  • Over 3 years of NP clinical experience.
  • Experience with ICD-10, HCC coding, and value-based care models.
  • Training in outpatient primary care, assisted living, or skilled nursing settings.

Skills & Abilities:

  • Strong communication skills.
  • Attention to detail and ability to work independently and in teams.
  • Customer service orientation.
  • Effective time management and prioritization skills.
