Nurse Practitioner, Adult and/or Gerontology

PHP Nurse Practitioners have a passion for delivering quality care to seniors and other residents in various healthcare settings, including skilled nursing facilities, assisted living, independent living communities, and via telehealth.

PHP is currently recruiting Nurse Practitioners for Clay Healthcare Center in Ivydale, WV. Our Nurse Practitioners work Monday - Friday, 8 am - 5 pm, with a salaried position that may require more than 40 hours some weeks. No call, weekends, or holiday coverage. Must be willing to work 5 days a week, Monday - Friday.

Job Summary:

The PHP NP provides high-quality primary care in a person-centered model, diagnosing, treating, and managing acute and chronic illnesses through wellness visits, early detection, and timely treatment. They collaborate with primary care providers and interdisciplinary teams, involving residents and families in care planning. This model aims to reduce hospital admissions and ER visits, focusing on customer satisfaction.

Job duties & responsibilities:

• Assess, diagnose, and manage conditions.
• Provide health education.
• Develop and communicate treatment plans.
• Order and interpret diagnostic tests, adjusting treatment as needed.
• Document appropriately in EMR.
• Collaborate with clinical leadership and support strategic initiatives.
• Ensure excellent patient experience and satisfaction.
• Practice ethically, following scope and standards, and comply with regulations.
• Coordinate care with other providers, including telemedicine.
• Complete regulatory visits and participate in family conferences and advanced care planning.
• Address pharmacy and medication recommendations.
• Be present in assigned facilities during work hours to ensure adequate face-to-face coverage.

Qualifications & Experience:

Required:

• MSN, DNP, or NP certification graduate.
• National certification (AANP or ANCC).
• Active, unrestricted state license.
• Active DEA license.
• US work authorization.
• 2+ years of clinical experience in geriatrics, adult, or family practice.
• Flexible schedule ability.
• Passion for senior and low-income populations.

Preferred:

• Geriatrics training or experience.
• 2+ years as a Nurse Practitioner.
• Experience with ICD-10, HCC coding.
• Experience in results-driven healthcare models.
• Willingness for professional development.
• Experience in value-based care, outpatient primary care, or assisted living settings.
