Responsibilities include comprehensive palliative visits, care planning with the interdisciplinary team, prescribing medications, documenting encounters in EMR, and coordinating with families and other healthcare professionals.

Qualifications

• Active Nurse Practitioner license in the state.
• Strong clinical assessment, documentation, and care planning skills.
• Experience with eCW EMR preferred (willing to train).
• Excellent communication and teamwork abilities.

• Conduct comprehensive palliative care visits, including assessment, diagnosis, and management of symptoms.
• Complete F2F encounters for hospice patients as needed.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Prescribe medications and coordinate care for patients with advanced or chronic illnesses.
• Accurately document all encounters in the EMR.
• Communicate effectively with patients, families, and other healthcare professionals.
