Job summary

Crossroads Hospice is seeking a Nurse Practitioner to join our compassionate palliative care team in Canton, OH. The role emphasizes patient- and family-centered care, with travel between visits and collaboration with a multidisciplinary team.

The ideal candidate has 2 years NP experience in acute care, Ohio NP license, DEA/NPI, and a MS in Nursing, plus a passion for home health and hospice care. Monday–Friday schedule with travel within the community.

Qualifications

• Active OH NP license.
• DEA number required.

• Current driver's license and auto liability coverage.

Responsibilities

• Provide patient- and family-centered palliative care and support.
• Travel between patient visits and coordinate care with the interdisciplinary team.
• Develop and implement individualized care plans in collaboration with patients, families, and clinicians.

Position Qualifications:

• 2 years of Nurse Practitioner experience in an acute care environment
• Home Health, Palliative Care, or Hospice Experience Required
• Masters of Science in Nursing
• Active unencumbered OH NP license
• Current driver's license and auto liability coverage
• Detailed assessment skills as part of this role require recertification for Hospice patients
• Interest in a community-based role that requires travel between patient visits
• Drug Enforcement Agency (DEA) number Required
• National Provider Identifier (NPI) Required
