Job summary

Interim HealthCare of Wausau is seeking a Nurse Practitioner to join a growing palliative care program. The role supports patients with complex illnesses in a patient-centered team setting, offering part-time and full-time schedules.

The NP must hold a Wisconsin license, provide strong assessment and care planning, and be comfortable with EMR documentation. Training is available for those early in their practice.

Qualifications

• Active Wisconsin NP license.
• Strong clinical assessment and care planning.
• Experience with EMR (eCW) preferred.
• Willing to train new NPs.

Responsibilities

• Conduct comprehensive palliative care visits, including assessment, diagnosis, and management of symptoms.
• Complete F2F encounters for hospice patients as needed.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Prescribe medications and coordinate care for patients with advanced or chronic illnesses.
• Document all encounters in the EMR.
• Communicate effectively with patients, families, and other healthcare professionals.
