Job summary

Fortis Health’s Select Home Health & Hospice sits in central Indiana and seeks a compassionate Hospice Nurse Practitioner to provide comfort, dignity, and high-quality palliative care in patients’ homes. You will collaborate with physicians and IDG members, develop and document care plans, and lead educational efforts for families and staff.

The position requires an Indiana NP or CNS certification, a valid driver’s license, and strong commitment to patient-centered hospice care.

Qualifications

• Minimum 5 years of nursing experience (preferred).
• Current NP or CNS certification in the state of Indiana
• Valid driver's license, auto insurance, and reliable transportation

Responsibilities

• Conduct required face-to-face hospice visits and report findings to the Hospice Medical Director
• Develop, coordinate, and document patient plans of care
• Collaborate with attending physicians and IDG members
• Prescribe medications (including controlled substances) within scope and delegation
• Order diagnostics and treatments as appropriate
• Educate patients, families, staff, and community on hospice and palliative care
• Participate in IDG and QAPI meetings
• Maintain accurate clinical documentation for compliance and reimbursement
• Supervise and evaluate home health aides as appropriate
• Complete required training modules
