Welltel in Brooklyn, NY is seeking a full-time Nurse Practitioner to join our in-office team. The role requires active NY Medicaid/Medicare numbers and an NP license in NY, with a four-day schedule of 10-hour shifts, Monday–Friday.

• EMR experience is required.

We are looking for a full time, experienced, Nurse Practitioner with anactive MEDICAID ID# IN NY! This role isin office at Brooklyn, NY(11219) with afour-day schedule (10 hours shifts), Mondays – Fridays.

Thepay range for the Nurse Practitioner position is: $150,000.00 - $181,000.00 per year.

Summary:

The Nurse Practitioner responsibilities include assessing the patients’ medical history and HPI, addressing patients' concerns regarding the use of telemedicine services and to provide personalized care to each patient.

The ideal Nurse Practitioner will be detail-oriented and able to communicate with patients effectively through various virtual communication channels.

Nurse Practitioner Job Duties and Responsibilities:

• Discussing and reviewing patients' medical history, symptoms, allergies, and current medications
• Asking patients situation-specific questions to make sure they get the care they need
• Formulating suitable treatment plans to address non-threatening conditions
• Prescribing various diagnostic tests to aid in providing accurate diagnoses
• Analyzing diagnostic test results and explaining them to patients
• Maintaining accurate records of patient's contact details, medical history, prescribed medications, allergies, diagnoses, and progress
• Advising patients on suitable diets and exercise regimens to improve their overall health.
• Assisting patients in managing chronic health conditions
• Recording accurate and timely notes in EMR
• Maintain accurate and timely documentation by completing all required forms and records before the end of each shift, in alignment with organizational standards and compliance guidelines

Requirements:

• Active NY Medicaid and Medicare # (Can start immediately)
• EMR experience
• Licensed NP/PA in NY
• Atleast 1 year experience in an urgent care setting or relative experience treating diverse patient population (patients of all ages) in a primary care setting. (Required)

• Strong attention to detail
