Qualifications

  • Current NP license and board eligibility as applicable.
  • BLS certification required.
  • DEA registration or ability to obtain it.
  • Minimum 2 years in primary care, family, internal, urgent care, or similar outpatient setting.
  • Willingness to practice across state lines where permitted.
  • Bilingual Spanish/English preferred.

Description

Hamilton Health Box (HHB) is a rapidly growing healthcare startup operating hybrid telemedicine-enabled primary care clinics for small and medium businesses across the country. Our goal is to find high potential individuals to drive a high-touch, low-cost healthcare experience for all of our patients and clients.

Position Summary: The Nurse Practitioner will work closely with the supervising Physician to manage several separate onsite employer clinics and rural health clinics and care for patients. It will consist of a combination of virtual and in-person medical care. The key objective is to develop positive relationships with key stakeholders and patients by creating value through onsite protocol-driven care, health coaching, disease management, preventative screening, and education. The NP will also help with urgent and emergent matters. The Nurse Practitioner will work closely with the supervising physician to help provide care. The Nurse Practitioner must also communicate with management and help design protocols to ensure the health and safety of patients and employees. The Nurse Practitioner will also help acquire data to determine population health management and identify areas for improvement in care and prevention. The NP will also help coordinate care and develop and implement screening protocols.

Duties & Responsibilities - Primary Care Advanced Practice Provider:
  • Promote and maintain patient health by providing medical services in collaboration with a physician.
  • Contribute to a physician's efficiency by identifying short-term and long-range patient care issues.
  • Recommend options and courses of action for patient care.
  • Implement physician directives.
  • Assess patient health by interviewing patients and performing physical examinations, including obtaining, updating, and studying medical histories.
  • Determine abnormal conditions by administering or ordering diagnostic tests, such as X-rays and laboratory studies, and interpreting test results.
  • Document patient care services by charting patient and department records.
  • Perform therapeutic procedures by administering injections and immunizations, suturing, and managing wounds and infections.
  • Instruct and counsel patients by providing normal growth and development information and counseling on emotional problems of daily living.
  • Provide a community of care by developing and implementing patient-management plans.
  • Maintain a safe and clean working environment by complying with procedures, rules, and regulations.
  • Protect patients and employees by adhering to infection-control policies and protocols. Comply with federal, state, and local legal and professional requirements by studying existing and new legislation, anticipating future legislation, enforcing adherence to requirements, and advising management on needed actions.
  • Collaborate in the development and evaluation of patient care. Update clinical guidelines/protocols/pathways; work autonomously and accept accountability for outcomes; sign orders in accordance with institution and state board requirements; collaborate in research and teaching projects; and apply research to practice.
  • Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.
  • Develop healthcare team staff by providing information, educational opportunities, and professional growth opportunities.
  • Utilize telemedicine interface to complete virtual visits.
  • Ability to float and round to multiple facilities.
  • Take calls by phone on a rotating schedule, generally one week on every 3–4 weeks. There are no after-hours onsite requirements while on call.
Requirements
Education/Experience:
  • Current licensure as a Nurse Practitioner (NP)
  • Maintenance of licensure and certification required
  • Active Basic Life Support (BLS)
  • Minimum 2 years of primary care, family medicine, internal medicine, urgent care, or comparable outpatient experience preferred/required
  • Current DEA registration or ability to obtain it, if required for the position
  • Bilingual Spanish/English preferred
  • Willingness to get licensure to practice in other states
