Qualifications

• Master's degree in Nursing required.
• Active Nursing License required.

Responsibilities

• Provides direct patient care relative to physical diagnosis and therapeutic management independently, using physician consultation where appropriate.
• Delivers care in subacute rehabilitation, long-term care, assisted living, rest homes, and home care environments.
• Performs assessment, planning, implementation, and evaluation of the plan of care for a panel of patients.
• Evaluates health status, including history, examinations, tests, and risk assessment to establish a diagnosis.
• Develops and implements care plans including treatments and medications within defined guidelines.
• Monitors response to therapy and revises the plan; educates and supports self-care.
• Collaborates with physicians and healthcare team; makes referrals as needed.
• Participates in quality and process improvement activities.

Job Details

• Provides direct patient care relative to physical diagnosis and therapeutic management independently, using physician consultation where appropriate
• Delivers care in one or a combination of settings including subacute rehabilitation, long-term care, assisted livings, rest homes, and home care environments
• Performs assessment, planning, implementation, and evaluation of the plan of care for a panel of patients
• Evaluates individual health care status including eliciting patient history, performing physical examinations, ordering screening and diagnostic tests, and risk assessment to establish a diagnosis
• Develops and implements plans of care including treatments and medications within defined guidelines for practice
• Monitors individual response to therapy and revises the plan as indicated, with attention to education, counseling, and encouragement of self-care
• Seeks consultation from the consulting physician and other members of the healthcare team and makes appropriate referrals as indicated by patient needs
• Collaborates with colleagues and community partners to develop effective patient care programs and maximize available multidisciplinary resources
• Participates in quality and process improvement activities to improve patient care and related systems
• Identifies learning needs of patients and staff and participates in interdisciplinary education programs including clinical precepting for nursing and medical students

• Master's degree in Nursing required
• Active Nursing License required
• Basic Life Support (BLS) certification required
• 1-3 years of related work experience required
• Valid prescriptive authority from both the MA DPH-DCP and Federal DEA required
• MA Board authorization to practice as an APRN NRSG required
• American Heart Association Basic Life Support Certificate required
• Experience with computer systems required, including web-based applications and Microsoft Office applications
