Job summary

Palm Primary Care, part of Palm Medical Centers, is seeking an experienced APRN to deliver senior primary care in our Texas centers. You will contribute to value-based care, coordinate with care teams, and support patients with chronic disease management.

The role emphasizes documentation, patient relationships, and quality outcomes, with a Monday–Friday schedule and relocation assistance as noted in benefits.

Qualifications

• Active Texas APRN license required.
• DEA license or eligibility within 2 months of hire.
• 2–3 years experience in primary care/value-based care setting with Medicare/Medicaid populations.
• Strong documentation and patient relationship-building skills.
• Proficient with EMR systems (eCW a plus).
• Knowledge of chronic disease management and quality metrics in value-based care.

Qualifications:

• Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
• Must have an active and unrestrictive TexasAPRN license
• Must have DEA license or eligible to obtain it within 2 months of date of hire
• Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
• Excellent documentation and patient relationship-building skills
• Proficient with EMR systems (eCW a plus!)
