Job summary

Qualifications

• Must have and maintain current licensure as a Nurse Practitioner or Physician Assistant in California.
• Current CPR certification is required.
• Must have or be willing to obtain DATA 2000 Waiver prior to employment.
• 2 years’ experience as a Nurse Practitioner or Physician Assistant.

Responsibilities

• Provides on-site chronic care clinics and sick visits as directed by site leadership.
• Delivers emergency treatment on-site and responds to urgent situations.
• Prescribes a comprehensive plan of care appropriate to patient needs.
• Adheres to policies and procedures to support medical care standards.
• Collaborates with Medical Director and Health Services Administrator to coordinate care.
