Atria Health Institute is launching its Los Angeles Institute and seeks Advanced Care Providers to join the internal medicine team serving adults in a high-quality preventive care setting.

You will conduct comprehensive assessments, diagnose acute and chronic conditions, and deliver primary and specialty care in collaboration with CMOs, while upholding our inclusive culture and performance standards. Responsibilities include home visits as needed and ongoing care coordination across services.

Qualifications

• CA NP license or CA PA license required
• 2+ years experience as NP/PA
• BLS & ACLS certification per American Heart Association

Responsabilités

• Assess patients with comprehensive history, exams, and diagnostic tests to identify health status and needs.
• Diagnose acute and chronic conditions and develop treatment plans.
• Provide primary and specialty care, order and interpret tests, and manage chronic diseases.
• Educate patients on health promotion, preventive care, and immunizations.
• Collaborate with interdisciplinary teams to coordinate care and refer as needed.
• Perform clinical skills such as phlebotomy, IV insertion, vaccines, and EKGs with patient records kept up to date.
