Job summary

Qualifications

• Master’s degree in nursing from an accredited school.
• Current NYS license to practice as a Registered Professional Nurse.
• Current NYS certification as a Nurse Practitioner (Adult/Geriatric).
• Maintains a current collaborative practice agreement.
• BCLS certification required.
• ACLS certification required as of 6/1/21 for NPs/PAs in CAMS & Cancer Center.
• Minimum two years’ experience in an acute care setting.

Responsibilities

• Understands and adheres to the WPH Performance Standards, Policies and Behaviors.
• Conducts patient rounds as necessary and formulates a therapeutic plan with physicians.
• Assesses daily care plans and recommends actions with the patient’s physician.
• Orders and interprets laboratory and diagnostic testing; maintains contact with testing areas.
• Performs regular medication reviews and modifies orders as needed.
• Recommends appropriate consults.
• Communicates plan of care and discharge plan with patient/family.
• Interacts with third party insurers regarding plan of care and continued stay.
• Participates in finalizing a safe discharge plan and completes all required documentation.
• Assists staff in moving patients along clinical pathways and serves as a resource to discharge planning staff.
• Inpatient rounding – assist physicians with pre-op and post-op assessments.
