Job summary

Hueman is hiring full-time Nurse Practitioners to perform comprehensive in-home health assessments for Medicare/Medicaid members in homes and SNFs. The role centers on preventive care, early detection, and closing care gaps, with autonomous field work and collaboration with PCPs when needed.

Visits are typically 60 minutes, Monday–Friday, with flexible scheduling and mileage reimbursement. Benefits include full medical/dental/vision and a 401(k) with PTO and holidays.

Qualifications

• Master’s degree or higher from an accredited NP program.
• Active NP license in the state of practice.
• Board Certification (AANP, ANCC, AACN).
• BLS certification and valid driver’s license.

Responsibilities

• Perform comprehensive adult and geriatric assessments during 60-minute in-home visits.
• Review medical history, reconcile medications, and identify care gaps.
• Conduct preventive screenings and point-of-care testing as applicable.
• Provide patient education and wellness counseling.
• Document findings accurately in EMR and coordinate with PCPs.
