Job summary

Optum is seeking a Nurse Practitioner for HouseCalls in Rock Island, IL. This field-based role delivers in-home preventive care to Medicare Advantage and other plans, focusing on education, gap closure, and collaboration with primary care providers.

Flexible scheduling allows 8am–8pm across a 7-day week, with 20–40 hours per week. External candidates may receive a $10,000 sign-on bonus, and multilingual candidates may earn additional compensation.

Qualifications

• Active NP certification (family, adult, geriatric, or adult-gerontology) and state licensure.
• Excellent communication with geriatric/Medicare populations.
• Experience with home health or in-home visits is a plus.

Responsibilities

• Perform in-home health assessments and reviews.
• Document findings and coordinate with PCPs to close care gaps.
• Educate patients on disease processes, medications, and self-care.
