Nurse Practitioner Job Details.
Location: King County, Washington; coverage may include Benton, Clark, King, Pierce, Whatcom, and Yakima counties.
Opportunity Type: Local, field-based Nurse Practitioner position with in-home patient visits.
Weekly Estimated Pay: $720-$860.
Start Date: November 2, 2026.
Duration: Approximately 52 weeks.
Schedule: 1 shift per week; approximately 8 hours per week.
Shift: 8-hour daytime shifts scheduled within operating hours of 8:00 a.m. to 8:00 p.m., seven days per week.
Daily Visit Volume: Approximately 6-7 visits during a full 8-or 10-hour day, depending on county needs.

Nurse Practitioner Job Requirements.
- At least 1 year of clinical experience; family practice, geriatric care, primary care, home health, or house-call experience is preferred.
- Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
- Active, unrestricted Nurse Practitioner license in Washington, or the ability to obtain Washington licensure by the start date.
- Active, unrestricted driver's license and reliable transportation for in-home patient visits.
- Ability to work independently in varied home environments and navigate stairs as needed.
- Ability to sit, stand, kneel, and transport clinical equipment weighing up to 30 pounds.
- Strong communication skills with geriatric and Medicare populations.
- Proficiency with electronic medical records and mobile healthcare technology.
- Ability to recognize urgent or emergent conditions and respond appropriately.
Home-Based Nurse Practitioner Responsibilities.
- Conduct comprehensive annual health assessments in patients' homes.
- Review medical histories, reconcile medications, obtain vital signs, and perform focused physical examinations.
- Complete evidence-based screenings and appropriate point-of-care testing.
- Identify and document diagnoses to support care management and individualized treatment planning.
- Recognize gaps in care and communicate findings to patients' primary care providers.
- Educate patients about disease processes, medications, preventive care, and treatment adherence.
- Address social determinants of health and connect patients' community resources.
- Identify urgent or emergent concerns and facilitate appropriate follow-up or escalation of care.
- Document patient visits accurately and efficiently in the electronic medical record.
- Collaborate with interdisciplinary care teams to help patients remain healthy and supported at home.
