Job summary

Greater Good Health is seeking a Nurse Practitioner to deliver advanced primary and nephrology-related care for our senior Medicare patients in Oregon. The role emphasizes time with patients, care planning, and collaboration with an integrated care team across dialysis and nephrology settings.

The ideal candidate holds an NP certification (ANCC/AANP), has 2+ years of clinical NP experience, and is licensed in Oregon.

Qualifications

• Must hold ANCC or AANP NP certification.
• 2+ years of clinical NP experience in primary care/gerontology/palliative care.
• Current DEA licensure and prescriptive authority after hire; CPR/BLS certification.

Responsibilities

• Support highly complex patients navigating the healthcare system with one-on-one care.
• Provide patient education, care plans, and clinical guidance to an integrated care team.
• Collaborate with Integrated Care team to deliver superior care for dialysis patients.
• Coordinate and facilitate high quality, cost-effective care with the healthcare team.
• Coordinate care between patient, family, physicians, and other providers.
• Perform tests and screenings; conduct comprehensive health and health risk assessments.
• Maintain HIPAA-compliant confidentiality.
• Address STAR gap closure.
