Greater Good Health is seeking a Nurse Practitioner to serve senior Medicare patients in Washington. This full-time role offers a hybrid schedule with a mix of in-clinic dialysis centers, nephrology practices, and in-home visits within your service area; telehealth may occur occasionally.

Qualifications

• Nurse Practitioner certification from ANCC or AANP.
• 2+ years of NP clinical experience in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care.
• Active DEA licensure/prescriptive authority post-hire.
• CPR/BLS certification.
• Knowledge/history working with HCCs.

Responsibilities

• Support highly complex patients, assisting them in navigating a challenging healthcare system and providing one-on-one patient care.
• Provide patient education, create care plans, and guide an integrated care team.
• Work with Integrated Care to improve care for dialysis patients.
• Coordinate and facilitate high-quality, cost-effective care with the healthcare team.
• Provide coordinated care between patient, family, physicians and other providers.
• Administer various tests and screenings.
• Perform comprehensive Health Assessments and Health Risk Assessments with careful clinical documentation.
• Maintain HIPAA confidentiality.
• STAR gap closure.
