Nurse Practitioner, HouseCalls - Statewide Traveler Idaho and Montana

Optum is seeking a Nurse Practitioner to join our HouseCalls team in Idaho and Montana. This role involves providing annual clinical assessments to patients in their homes, helping to identify health risks and coordinate follow-up care. The position offers flexible scheduling, no on-call, weekends, or holidays, and requires travel across Idaho and Montana (approximately 85%). All travel expenses are covered, with a base salary plus incentives.

Primary Responsibilities:

• Conduct in-home assessments including medical history, review of systems, medication reconciliation, vital signs, and physical exams.
• Perform screenings and point of care testing as appropriate.
• Identify diagnoses for care management and treatment.
• Communicate findings to the patient's PCP and identify urgent situations.
• Educate members on health topics.

Required Qualifications:

• Active NP license or ability to obtain one by start date, free of disciplinary actions.
• Active ANCC or AANP certification in relevant specialties or ability to obtain it.
• Ability to perform physical tasks like moving equipment and navigating various dwelling conditions.
• Driver's license and reliable transportation for travel.

Preferred Qualifications:

• 1+ years of clinical experience.
• Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
• Home health or home visit experience.

This role offers a salary range of $89,800 to $176,700 annually, with benefits, incentives, and coverage of travel expenses. The position requires a minimum 12-month commitment, with full benefits and opportunities for career development.
