Job Opportunity: Nurse Practitioner - Home and Community Care

$40,000 Student Loan Repayment or $25,000 Sign-on Bonus for individuals new to this program.

Optum is seeking a Nurse Practitioner to join our HouseCalls team in Idaho and Montana. We are a clinician-led care organization dedicated to creating a seamless health journey for patients across various care settings.

As part of our Home and Community Care team, you'll conduct annual clinical assessments in patients' homes, helping to identify health risks and coordinate follow-up care to enhance well-being.

This role offers work-life balance with flexible scheduling over a 7-day week, no on-call, weekends, or holidays required. Travel across Idaho and Montana is approximately 85%, with all expenses covered. Compensation includes a base salary plus incentives.

Primary Responsibilities:
  1. Conduct in-home assessments covering medical history, review of systems, medication reconciliation, vital signs, and physical exams.
  2. Perform evidence-based screenings, including point of care testing as appropriate.
  3. Identify diagnoses for care management and active medical treatment.
  4. Document medical conditions based on assessments and clinical judgment.
  5. Communicate findings to inform members' PCPs and identify care gaps.
  6. Recognize urgent/emergent situations and intervene accordingly.
  7. Educate members on disease processes, medications, and compliance.
Required Qualifications:
  • Active NP license or ability to obtain one by start date, free of disciplinary actions.
  • Active national certification in relevant specialties or ability to obtain certification and licensure in the assigned state.
  • Physical ability to perform assessments, including moving equipment and navigating various dwelling conditions.
  • Valid driver’s license and reliable transportation for travel within licensed states.
Preferred Qualifications:
  • At least 1 year of clinical experience in relevant settings.
  • Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
  • Home health or home visit experience.
  • Potential requirement for additional licensure in other areas.

Note: Employees must stay in role for at least 12 months; leaving earlier will stop student loan repayment benefits. Full benefits are contingent upon remaining in the role for 36 months.

The salary range is $89,800 to $176,700 annually, based on experience, education, and location. Benefits include comprehensive health plans, incentives, stock options, and 401(k).
