Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) for in-home preventive care with the HouseCalls team in Georgia. This field-based role focuses on assessments, education, and care coordination to help members stay healthy at home.

You will conduct annual wellness visits, identify gaps in care, and collaborate with primary care providers, with a flexible schedule (8:00 AM–8:00 PM, 20–40 hours/week) and no on-call, weekend, or

• Physical assessments and vital signs
• Preventive screenings and point-of-care testing when appropriate
• Identify and document clinical findings that support treatment planning and care management

• Recognize urgent clinical concerns and take appropriate action
• Educate members on chronic conditions, medications, and healthy lifestyle practices
• Address social determinants of health and connect members to community resources when needed

About the Role

HouseCalls is a unique in-home assessment program focused on delivering preventive health services directly to members. As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you'll conduct annual wellness visits in patients' homes, helping identify care gaps, improve health outcomes, and communicate important findings to primary care providers. This is a non-prescribing, field-based position centered on assessment, education, and care coordination.
