Job summary

Del Corazon is seeking a Hospice Nurse Practitioner to conduct in-home visits across Albuquerque, NM, working with our Medical Director to coordinate comprehensive patient care. You will assess symptoms, adjust care plans, and provide emotional support to patients and families in end-of-life care settings.

Ideal candidates hold a Master’s or Doctorate in Nursing and NM NP licensure, with 1+ year hospice experience and APRN certification.

Qualifications

• Master’s degree or Doctorate in Nursing, with current NP licensure in New Mexico
• Minimum 1 year of NP experience in hospice or palliative care
• Current APRN certification required

Responsibilities

• Conduct face-to-face visits with hospice patients in their homes or facilities to assess and support clinical needs
• Collaborate with the hospice Medical Director and other healthcare professionals to ensure optimal patient care and continuity
• Evaluate and manage patient symptoms, adjust care plans as needed, and provide emotional support to patients and families
• Document patient encounters accurately and timely to ensure regulatory compliance
• Serve as a resource for clinical questions and assist in educating families and caregivers about the end-of-life process and available resources

Join our compassionate hospice team at Del Corazon, as a Nurse Practitioner (NP) providing essential support and care to our patients and their families. We are seeking a skilled NP to conduct face-to-face visits and collaborate closely with our hospice Medical Director to ensure comprehensive patient care.

• Skills: Strong clinical assessment and communication skills, with an empathetic approach to patient care
• Transportation: Reliable transportation and valid auto liability insurance (mileage reimbursement provided)
