Job summary

Circle of Life Hospice seeks an NP/ACNP with unrestricted Arkansas licensure, and RN licensure, to join our inpatient/hospice team. The role requires 3+ years in inpatient or nursing facility settings, with preference for hospice experience.

Certification by ANCC or AANP is required, and Oklahoma compact licensure is anticipated. The position involves patient assessments, care coordination with physicians, and communications with families about hospice care, costs, and services.

Qualifications

• AR APN/NP licensure required; RN licensure current and active.
• 3+ years in inpatient, nursing facility, internal medicine or family practice.
• Hospice experience preferred.
• Current ANCC or AANP certification; valid driver's license.

Responsibilities

• Performs assessments to determine hospice admission, recertification, and ongoing patient care needs; reports results to Physician team.
• Provides information to patients, families about hospice philosophy, goals, reimbursement and services.
• Ensures timely communication with community physicians to promote continuity of care.
• Interprets and records data from diagnostics, orders studies as appropriate; provides subsequent care.
• Arranges consultations from subspecialties when needed; administers medications as permitted.

Job description

Qualifications: Unrestricted APN/NP licensure in State of Arkansas with ability to attain compact licensure for Oklahoma coverage. Three or more years of experience working in an inpatient or nursing facility, internal medicine or family practice environment required. Experience in a hospice program preferred. License/Certification: Registered Nurse licensure, with current approval to practice as a Nurse Practitioner by the State of AR. Current certification through the ANCC or AANP. Valid driver's license with an automobile that is insured in accordance with state registration requirements.

• Performs a variety of assessments to determine the appropriateness for hospice admission, re-admission/recertification, and any other assessments needed to maintain ongoing patient care. Reports assessment results to Physician team. Participates in Education and other Nursing related tasks as needed (patient care and administrative).Maintains a professional and helpful presence when visiting patients in nursing facilities, hospitals, serving as a resource to medical professionals concerning hospice programs, palliative care and pain management.
