Qualifications

• Active APRN license in Arkansas with ability to obtain compact licensure for Oklahoma.
• Three or more years in inpatient, nursing facility, internal medicine or family practice.
• Hospice program experience preferred; prescriptive authority documented.
• Driver's license with valid auto insurance.

Responsibilities

• Performs assessments for hospice admission, re-admission/recertification, and ongoing patient care; reports results to Physician team.
• Provides information to patients and families about hospice philosophy, goals, and services.
• Ensures communication with community physicians to promote continuity of care.
• Interprets and records data from diagnostics; orders studies as appropriate.
• Provides subsequent care with assessment of condition and response to care.
• Arranges consultations from subspecialty services when needed.
