Del Corazon Hospice in Los Alamos, NM, is seeking a Nurse Practitioner to perform face-to-face hospice visits, collaborate with the Medical Director, and manage patient symptoms with compassionate care.
The ideal candidate has an NP degree (MSN/NP or DNP), NM licensure, and at least 1 year of hospice/palliative care experience, plus APRN certification and strong communication skills.
Join our compassionate hospice team at Del Corazon, as a Nurse Practitioner (NP) providing essential support and care to our patients and their families. We are seeking a skilled NP to conduct face-to-face visits and collaborate closely with our hospice Medical Director to ensure comprehensive patient care.
Conduct face-to-face visits with hospice patients in their homes or facilities to assess and support their clinical needs
Collaborate with the hospice Medical Director and other healthcare professionals to ensure optimal patient care and continuity
Evaluate and manage patient symptoms, adjust care plans as needed, and provide emotional support to patients and families
Document patient encounters accurately and timely to ensure regulatory compliance
Serve as a resource for clinical questions and assist in educating families and caregivers about the end-of-life process and available resources
Education: Master’s degree or Doctorate in Nursing, with current NP licensure in New Mexico
Experience: Minimum of 1 year of experience as an NP in a hospice or palliative care setting
Certification: Current Advanced Practice Registered Nurse (APRN) certification
Skills: Strong clinical assessment and communication skills, with an empathetic approach to patient care
Transportation: Reliable transportation and valid auto liability insurance (mileage reimbursement provided)
