Job summary

Qualifications
- Master’s NP degree and active MA NP license required.
- National board certification (AANP/ANCC/ONCC) preferred.
- Ability to perform home visits with chronic illness management.
Responsibilities
- Conduct home-based patient evaluations and comprehensive health assessments.
- Diagnose conditions, develop care plans, and prescribe appropriately.
- Coordinate care with physicians and specialists for integrated treatment.
- Educate patients and families on disease management and prevention.
- Maintain accurate EHR documentation of visits and follow-ups.
- Monitor patient progress and adjust treatment as needed.
- Respond to emergencies during home visits and refer when necessary.
- Engage with community to promote health services.
