Job summary

Essen Health Care is seeking a compassionate Nurse Practitioner to provide patient-centered care through home visits in the Greater Boston region. The role emphasizes holistic care, education, and management of complex conditions, with travel across Massachusetts as needed.

Qualifications

• Master’s NP degree and active MA NP license required.
• National board certification (AANP/ANCC/ONCC) preferred.
• Ability to perform home visits with chronic illness management.

Responsibilities

• Conduct home-based patient evaluations and comprehensive health assessments.
• Diagnose conditions, develop care plans, and prescribe appropriately.
• Coordinate care with physicians and specialists for integrated treatment.
• Educate patients and families on disease management and prevention.
• Maintain accurate EHR documentation of visits and follow-ups.
• Monitor patient progress and adjust treatment as needed.
• Respond to emergencies during home visits and refer when necessary.
• Engage with community to promote health services.

Job Summary

Intention Healthcare is seeking a compassionate and skilled Nurse Practitioner (NP) to provide high-quality, patient-centered care through home visits in the Greater Boston region of Massachusetts. This role focuses on individuals with chronic illnesses, mobility challenges, or limited access to healthcare facilities. The NP will emphasize holistic care, patient education, and management of complex medical conditions, ensuring continuity of care for patients in their homes.

Responsibilities

• Patient Evaluations Conduct comprehensive health assessments during home visits, including physical exams, medication reconciliation, and review of medical histories.


• Diagnosis & Treatment Diagnose medical conditions, develop individualized care plans, and prescribe medications as appropriate.


• Care Coordination Collaborate with physicians, specialists, and other healthcare professionals to ensure integrated and continuous care.


• Patient Education Educate patients and families on disease management, lifestyle changes, medication adherence, and preventive care.


• Documentation Maintain detailed and accurate electronic health records (EHR) of patient assessments, treatments, and follow-ups.


• Follow-Up Care Monitor patient progress through follow-up visits and adjust treatment plans as necessary.


• Emergency Response Assess and manage urgent medical situations during home visits, providing timely interventions or referrals.


• Community Engagement Participate in local outreach initiatives to promote health services and educate the community about available resources.

Job Requirements

• Master’s Degree in a Nurse Practitioner program (Family or Adult Geriatrics).


• Current Massachusetts Nurse Practitioner license required.


• National Board Certification through one of the following

• Bilingual (Spanish-speaking) is a plus.
