Job summary

NPHire is seeking a Nurse Practitioner to deliver in-home primary care across the Greater Boston region. This role involves comprehensive assessments, care coordination, and treatment planning for medically complex patients in patients' homes.

Responsibilities include managing chronic conditions, medication reconciliation, EMR documentation, and patient education. Travel throughout Greater Boston with occasional evenings or weekends is required.

Qualifications

• Master's degree in Nurse Practitioner (Family or Adult-Gerontology).
• Active Massachusetts NP license or eligibility.
• National certification through AANP, ANCC, or ONCC.
• Experience with EMR/EHR systems preferred.
• Spanish bilingual is a plus.

Responsibilities

• Provide comprehensive home-based primary care throughout the Greater Boston region.
• Care for patients with chronic illnesses, mobility limitations, and complex medical conditions.
• Conduct comprehensive in-home health assessments, physical exams, medication reconciliation, and medical history reviews.
• Diagnose and treat acute and chronic medical conditions.
• Develop individualized treatment plans and prescribe medications as appropriate.
• Coordinate care with primary care physicians, specialists, and multidisciplinary healthcare teams.
• Educate patients and caregivers on disease management, preventive care, medication adherence, and lifestyle modifications.
• Perform follow-up visits and adjust treatment plans based on patient progress.
• Assess urgent medical situations during home visits and coordinate appropriate interventions.
• Maintain timely and accurate electronic medical records.
• Participate in community outreach and health education initiatives as needed.
• Travel throughout the Greater Boston area to provide patient care.
• Work flexible schedules with occasional evenings or weekends based on patient needs.
