Job summary

The Nurse Practitioner at People’s Health conducts clinical assessments of PH members, documents encounters in the EMR, and works to improve our plan member's quality of life and health outcomes through cost-effective care.

This role requires in-home visits, travel within a designated area, Louisiana licensure, NP certification, prescriptive authority eligibility, BLS, and strong patient education skills.

Qualifications

• Active, unencumbered RN license in the assigned state.
• NP certification from ANCC/AANP/AACN as Adult, GerIatric, Family or Acute Care NP.
• Prescriptive authority in Louisiana or ability to obtain post-hire.
• Willing to spend time with members in their homes, including remote areas.
• Willing to move a 30-pound bag and navigate stairs in varied homes.
• Current BLS certification or ability to obtain before employment.
• Valid driver's license with reliable transportation for travel to sites.
• Home care or home visit experience.
• 1+ years of clinical experience in highest level of education.

Responsibilities

• Conduct assessments in member homes and clinics with emphasis on documentation.
• Check vitals and perform physical exams.
• Identify diagnoses for care management and active medical management.
• Formulate current/past medical condition lists from assessments.
• Communicate findings to inform PCP of care gaps.
• Educate members on disease processes, medications, and compliance.
• Initiate appropriate referrals.
• Comply with HIPAA and protect PHI.
• Autonomous work in various environments, including geriatric care.
