Qualifications
- Valid license to practice in the state of employment (NP/PA/MD) in good standing.
- Experience in home health, clinic, facility or telehealth preferred.
- Strong communication and teamwork skills.
Responsibilities
- Evaluate high-risk patients and develop individualized care plans.
- Coordinate care with interdisciplinary team to ensure continuity.
- Communicate with case managers, clinicians, pharmacists, and health coaches.
- Document care activities accurately for compliance.
- Provide patient education and self-management resources.
- Travel to patients' homes and facilities for visits.
- Adhere to HIPAA and Medicare guidelines.
- Monitor quality measures and contribute to care improvements.
