Responsibilities

• Manage acute and chronic medical conditions for a frail elderly population.
• Collaborate with physicians, nurses, specialists, and the interdisciplinary care team.
• Provide patient and family education regarding treatment plans and disease management.
• Coordinate specialty referrals, hospital transitions, and follow-up care.
• Maintain accurate and timely clinical documentation.
