Job summary

Protouch Staffing seeks an experienced Nurse Practitioner or Physician Assistant to deliver comprehensive primary care to seniors enrolled in PACE in Bakersfield, CA. You will work with the Medical Director and care team across clinic, home, hospital, nursing facility, and community settings to promote independence and improve quality of life for older adults.

The role requires an accredited NP/PA program graduate with CA licensure, DEA, and 5+ years of clinical/geriatric experience.

Qualifications

  • Must be a graduate of an accredited NP/PA program.
  • Current, unrestricted California NP or PA license.
  • DEA registration or ability to obtain within six months.
  • Minimum 5 years of clinical nursing or advanced practice experience.
  • At least 1 year of geriatric care experience required.
  • Experience in PACE or outpatient/community health preferred.
  • Experience in interdisciplinary teams is preferred.
  • CPR/BLS certification.

Responsibilities

  • Perform comprehensive health assessments and develop individualized care plans.
  • Diagnose and manage acute and chronic conditions, order tests, and prescribe medications.
  • Collaborate with physicians, nurses, and specialists to ensure coordinated care.
  • Provide primary care in clinic, homes, hospitals, and community settings as needed.
  • Monitor progress, adjust treatment plans, and educate patients and caregivers.
  • Maintain accurate EMR in compliance with standards.
  • Participate in meetings, quality improvement, and case conferences.
  • Ensure adherence to clinical standards and patient safety protocols.

Key Responsibilities
  • Perform comprehensive health assessments, physical exams, and periodic reassessments while developing individualized care plans.
  • Diagnose and manage acute and chronic conditions by ordering/interpreting diagnostic tests, prescribing medications, and performing procedures within scope of practice.
  • Collaborate with physicians, nurses, specialists, caregivers, and the interdisciplinary team to ensure coordinated, continuous care.
  • Provide primary care services in clinic, participant homes, hospitals, nursing facilities, and community settings as needed.
  • Monitor participant progress, adjust treatment plans, provide patient/caregiver education, and promote preventive care.
  • Maintain accurate and compliant electronic medical records (EMR).
  • Participate in interdisciplinary meetings, quality improvement initiatives, and case conferences.
  • Ensure compliance with clinical standards, regulatory requirements, and patient safety protocols.
Qualifications
  • Graduate of an accredited Nurse Practitioner or Physician Assistant program.
  • Current, unrestricted California NP or PA license.
  • DEA registration (or ability to obtain within six months of hire).
  • Minimum 5 years of clinical nursing or advanced practice experience preferred.
  • At least 1 year of experience caring for frail, elderly, or geriatric populations required.
  • Experience in PACE, primary care, geriatrics, outpatient care, home health, or community health preferred.
  • Experience working within an interdisciplinary healthcare team preferred.
  • Supervisory or leadership experience is a plus.
  • Current BLS/CPR certification.
