Optum Home & Community Care, part of Optum, is hiring an Advanced Practice Clinician to support high‑risk members in Senior Community Care settings. The role focuses on assessments, rehab support, documentation, and collaborative care planning to improve care transitions.

Qualifications

• Active ANCC/AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care.
• Active, unrestricted Texas license or ability to obtain by start date.
• 1+ year of clinical APC experience in long-term care or similar.
• Active DEA licensure or ability to obtain post-hire.

Responsibilities

• Conduct comprehensive assessments, including past medical history, meds, vital signs and exams.
• Support members during short-term sub-acute rehabilitation episodes.
• Ensure accurate documentation, including ICD-10 conditions.
• Collaborate with interdisciplinary care team.
• Conduct advanced illness and advanced care planning conversations.
• Provide counseling and education to patients and caregivers.
